Gout

Overview

Gout is acute inflammatory arthritis caused by uric acid crystals — classically sudden, exquisite first-MTP (big toe) pain that wakes you from sleep. Acute attacks are treated with indomethacin or naproxen, colchicine, or a short prednisone burst. Clindle treats acute gout same-visit, then for patients with recurrent attacks or tophi, starts allopurinol titrated to a target uric acid <6 mg/dL. We screen for kidney disease and adjust dosing. Lifestyle (alcohol, fructose, purine-heavy diet, weight) matters.
