Our commitment
Clindle and its affiliated treating clinicians are committed to protecting the privacy of your Protected Health Information ("PHI") as required by the Health Insurance Portability and Accountability Act ("HIPAA") and applicable state law.
How we may use and disclose your PHI
- Treatment — to provide, coordinate, and manage your care, including sending prescriptions to your pharmacy and orders to labs and imaging centers.
- Payment — to bill your insurance, collect self-pay charges, and obtain prior authorization.
- Operations — quality review, training, and business operations of the practice.
- As required by law — public health reporting, court orders, and other legal obligations.
Uses requiring your authorization
Marketing, sale of PHI, and most disclosures of psychotherapy notes require your written authorization. You can revoke authorization at any time in writing.
Your rights
- Inspect and copy your records.
- Request corrections.
- Request restrictions on certain uses and disclosures.
- Request confidential communications.
- Receive an accounting of disclosures.
- Receive a paper copy of this Notice.
- Be notified of a breach affecting your PHI.

You can complain to us at care@clindle.com or to the U.S. Department of Health and Human Services. We will not retaliate for filing a complaint.
