FMLA paperwork
Family and Medical Leave Act forms must be completed by a provider with an ongoing treatment relationship.
Short- or long-term disability forms
Disability certifications require longitudinal records that single-visit care can't provide.
ADA workplace accommodations
ADA documentation requires a treating clinician familiar with your condition over time.
Controlled substance prescriptions
We don't prescribe Adderall, Xanax, testosterone, opioids, or other DEA-scheduled medications.
Everything else, we've got you.
