Insomnia means trouble falling asleep, staying asleep, or early-morning waking at least three nights a week for three months, with daytime impact. First-line treatment is cognitive behavioral therapy for insomnia (CBT-I) — more effective long-term than medication. Clindle screens for treatable drivers (anxiety, depression, restless legs, sleep apnea, thyroid disease, late-day caffeine), prescribes non-controlled options like trazodone, doxepin, hydroxyzine, ramelteon, or melatonin when needed, and refers for CBT-I and sleep studies. We do not prescribe controlled sleep medications like Ambien or benzodiazepines over telehealth.
