Overview
LillyDirect Self Pay Pharmacy Solutions is Eli Lilly's direct-to-patient pharmacy for people paying cash for Zepbound. Instead of the pre-filled KwikPen, LillyDirect ships single-dose vials plus syringes — the exact same tirzepatide medication at a lower price.
Treatment
Current LillyDirect self-pay pricing (2026): 2.5 mg starter vial $349/month, 5 mg $499/month, 7.5 mg $599/month, 10 mg $699/month. 12.5 and 15 mg vials are not currently offered on LillyDirect — those doses require the KwikPen. Prescriptions must come from a licensed U.S. clinician; Clindle can send yours directly to LillyDirect Pharmacy Solutions after your online visit.
When to book a visit

Are LillyDirect vials the same as Zepbound KwikPen?
Same tirzepatide, same FDA approval, same manufacturer. The only difference is packaging (single-dose vial + syringe vs pre-filled auto-injector).
Can I use insurance with LillyDirect?
No — LillyDirect Self Pay is a cash-only channel. If you have insurance coverage for Zepbound, fill at a standard pharmacy and use the Savings Card instead.
Why aren't higher doses on LillyDirect?
Lilly currently reserves the 12.5 mg and 15 mg strengths for KwikPen production. Most patients reach a maintenance dose at 10 mg and can stay on the LillyDirect vials long-term.
