Overview
Diet lowers LDL 10–15% at best — meaningful, but not a substitute for statins in high-risk patients. Focus on soluble fiber, unsaturated fats, and reducing saturated fat.
Treatment
Highest-evidence foods: oats and barley (soluble fiber), legumes, nuts (30 g/day), fatty fish (2x/week), olive oil, plant sterols (fortified spreads), soy. Reduce butter, cheese, red meat, tropical oils. Consider statins when LDL >190, ASCVD risk >7.5%, or diabetes.
